AB OBJECTIVE: To describe manifestations of CMV gastrointestinal disease
and to evaluate correlations between CMV infection and non-typhi
Salmonella sepsis. METHODS: Retrospective chart review by clinicians
following the patients prospectively. RESULTS: 8 AIDS patients (four
homosexuals, three intravenous drug abusers, one heterosexual) developed
CMV gastrointestinal disease. The patients were recovered at our
institution for fever and abdominal discomfort. 6 patients had
Salmonella spp (non-typhi) sepsis. Of these patients, two had intestinal
perforation: biopsies performed during surgical operation revealed CMV
intestinal disease, with lesions localized in the appendix in one case
and in the caecum in the other. The other four patients were evaluated
by endoscopy. In two of them, gastroscopy revealed large gastric
biopsy-proven CMV ulcers. In the other two cases, large esophageal CMV
polypoid lesions and ulcers were found. Of the last two patients, one
was evaluated for perianal ulcers and sigmoidoscopy showed multiple CMV
ulcers between anal tract and rectum; the other for disphagia, and
endoscopy showed esophageal CMV ulcers. CONCLUSIONS: our observations
suggest an association between Salmonella sepsis and active CMV
gastrointestinal infection.
